3. $26 Million Fraud by Texas Attorney Tshombe Anderson 

In 2018, Attorney Tshombe Anderson was able to defraud the Federal Workers’ Compensation Program to the tune of over 26 million. He did this by stealing patient information and submitting fraudulent claims. He was, however, caught and sentenced to imprisonment for 10 years.

4. 7.8 Million Premium Evasion by Parkside Construction 

Parkside Construction and Affinity Human Resources, located in New York, underreported 42 million in payroll and underpaid workers by 1.7 million, resulting in a 7.8 million evasion in workers’ compensation insurance premiums. Their scam was discovered in  2018, and they paid for their crimes.

5. 6 Million Fraud by Enrique and Gloria Vera 

In 2018, the siblings Enrique and Gloria Vera were convicted for underpaying their employees. They owned a construction company and defrauded the California State Compensation Insurance Fund of at least 6 million because they altered their payroll records.

6. 5 Million Kickback Scheme by Pharmacist Nermin Awad El-Hadik

In 2018, pharmacist Nermin Awad El-Hadik was convicted for his kickback scheme involving illegal payments for workers’ compensation referrals. They were over 5 million in fraudulent claims, and his fraud was discovered.

7. 4.4 Million False Billing by Dr. Todd Sloan Koppel 

Dr. Todd Sloan Koppel was convicted for submitting over 4.4 million in fraudulent bills to insurance companies. This led to the suspension of his authorization to treat injured workers. 

8. 3.2 Million Premium Fraud by Security Code 3, Inc. 

Troy Carson was the owner of Security Code 3, Inc. He was later charged with underreporting payroll by more than 12 million. It was discovered that it resulted in over $3.2 million in workers’ compensation fraud.
